Tube Layout Label for 48-200 Code 121

Would anybody have a good scan of a Tube Layout Label for 48-200 Code 121 that is affixed to the underside of the cabinet? thanks

There's a decent picture at RadioMuseum:

Obviously it would need to be corrected for perspective and cleaned up.
