This can't be correct.  The alignment instructions on Nostalgia air say to hook the output meter up to the plate and cathode of the 42 tube for alignment.   Since the cathode is grounded this just reads the plate voltage.  So do I hook it up between the plate and grid (pins two and three of the 42 tube)?

It is correct. You do want plate voltage to Chassis/cathode. You should use AC setting for that, you are not interested in the DC.

Got it! The last radio I aligned I was measuring avc voltage so I had the meter on DC.

It doesn't matter, you could put the leads at the output transformer secondary, across the voice coil, also on AC, or measure DC on AVC voltage. They are all good indicators.
